Assumption of Risk
I understand that dance involves physical activity and inherent risks, including but not limited to slips, falls, muscle strains, sprains, and other injuries. I voluntarily assume all risks associated with participation in classes, rehearsals, performances, or events at La Bodega Dance Studio. I hereby release La Bodega Dance Studio, its owners, instructors, employees, and affiliates from any and all claims, liabilities, or demands arising from injury, illness, or damage sustained while participating in any activities at the studio.
Health & Medical Disclaimer
I confirm that I am in good physical condition and have consulted a physician if necessary before participating. I agree to inform the instructor of any medical conditions or limitations that may affect my ability to dance safely. In case of an emergency, I authorize La Bodega Dance Studio to seek medical treatment for myself (or my child, if applicable). I accept responsibility for any medical expenses incurred.
Studio Rules & Conduct
We expect students to treat others with respect and equality. Students must not disrupt class or put others down. I agree to follow all studio rules and instructions from staff. Failure to do so may result in removal from class without refund. Studio must be left in its original condition. Please take all belongings. Any lost and found items will be kept for a month until its donated.
Cancellation/Tardy Policy
I understand that:
Canceled classes by the studio will result in a full refund/ticket transfer.
No refunds will be given for no-shows or same day cancellations.
We do not accept students more than 30 minutes late to class.
Student Responsibilities
I agree to be financially responsible for any intentional or negligent damage caused by myself (or my child) to the studio’s property, including but not limited to:
Mirrors, floors, walls, or sound equipment.
Furniture, waiting areas, or restrooms.
Any other studio-owned materials or facilities
Photo/Video Release
By signing this agreement, I hereby release La Bodega Dance Studio, its owners, instructors, employees, and affiliates from any and all claims, liabilities, or demands regarding photo/video release. I understand that my participation in any classes will have a filming block at the last 15-20mins, whether its group videos or BTS. Filming is an OPTIONAL choice, anyone not interested can standby or watch from our lobby area! These efforts are to stop any media from being publicized without the customer's knowledge. La Bodega Dance Studio is NOT responsible for any media released by any other student.
Prohibited Activities
No smoking, THC pens, vapes, or cigarettes
No alcohol
No FOOD. NO COFFEE. ONLY WATER!
No illegal substances.
You agree to the terms listed above by signing this waiver. This agreement is governed by Illinois law. Disputes will be resolved in Cook County courts.
