Studio Policy & Photoshoot Waiver

Assumption of Risk I understand that dance involves physical activity and inherent risks, including but not limited to slips, falls, muscle strains, sprains, and other injuries. I voluntarily assume all risks associated with participation in classes, rehearsals, performances, or events at La Bodega Dance Studio. I release La Bodega Dance Studio, its owners, instructors, employees, and affiliates from any claims, liabilities, or demands arising from injury, illness, or damage sustained while participating in studio activities. Health & Medical Disclaimer I confirm that I am in good physical condition and have consulted a physician if necessary before participating. I agree to inform the instructor of any medical conditions or limitations that may affect my ability to dance safely. In case of emergency, I authorize La Bodega Dance Studio to seek medical treatment for myself (or my child, if applicable) and accept responsibility for any medical expenses incurred. Studio Conduct Treat others with respect and equality. Disruptive behavior or putting others down will not be tolerated. Failure to do so may result in removal from class without refund. All belongings must be taken. Lost and found items will be kept for one month before donation. Cancellation/Tardy Policy Canceled classes by the studio will result in a full refund or ticket transfer. No refunds will be given for no-shows or same-day cancellations. Students arriving more than 30 minutes late will not be admitted. Student Responsibilities I agree to be financially responsible for any intentional or negligent damage caused by myself (or my child) to studio property, including mirrors, floors, walls, sound equipment, furniture, waiting areas, restrooms, and any other studio-owned materials or facilities. Photo/Video Release By signing this agreement, I release La Bodega Dance Studio, its owners, instructors, employees, and affiliates from any claims, liabilities, or demands related to photo/video recording. I understand that classes may include a 15–20 minute filming session at the end for group or behind-the-scenes content. Participation is optional; those not interested may wait or observe from the lobby. The studio is not responsible for media released by other students. Prohibited Activities: Smoking, Weapons on site, Alcohol, and/or Food. By signing you agree to all terms listed above. This agreement is governed by Illinois law, and disputes will be resolved in Cook County courts.
