Studio Policy & Photoshoot Waiver

I, the undersigned, agree to the following terms to attend or participate in any rental, class, rehearsal, or event at La Bodega Dance Studio. By signing, I confirm I have read and understood these terms. Violation of any policy may result in removal from the premises and permanent denial of entry. Rental & Attendance Rules: All bookings must include setup and cleanup time. Studio staff may assist with setup but are not responsible for cleanup. Cleanup is the renter’s responsibility. An overtime fee of $1 per minute applies if the booking exceeds the scheduled time by more than 15 minutes. Every person attending must complete this waiver. No waiver, no entry. Tardy Policy: A 15-minute grace period is allowed with notice. Without notice, the space may be released to the next booking. Missed time due to tardiness cannot be transferred to future rentals. Extensions are not permitted if another booking follows. Media Release: I acknowledge security cameras are in use. I grant La Bodega Dance Studio permission to photograph or record me and release the studio, its owners, staff, and affiliates from all claims related to photo or video use. Media may be used for promotional purposes. Credit is not guaranteed. Cancellation Policy: Full refund if canceled at least 48 hours in advance. A $30 fee applies for Studio A and a $12.50 fee for Studio B if canceled same day. No refunds for same-day cancellations. Responsibilities: I am responsible for all damages caused by myself or my guests. A $30 cleaning fee applies if the studio is not left in its original condition. Renters are responsible for supervising all attendees. Minors must be supervised at all times. Liability Waiver: I acknowledge physical activity involves risk of injury and voluntarily assume all risks. I waive all claims against La Bodega Dance Studio except in cases of gross negligence. I agree to cover legal costs arising from my actions or event. Prohibited Activities: No smoking, vaping, THC pens, or cigarettes. No alcohol. No food or drinks. Water only. No coffee. No adhesives, tapes, sprays, or substances that may damage studio property. Acknowledgment: I confirm I am 18 or older, or my parent/guardian has signed on my behalf, and I agree to all terms listed above.
